This form is intended to standardize the way in which applicants of wind facilities request copies of site plan approvals (issued under section 41 of the Planning Act) and building permits (issued under s. 8 of the Building Code Act, 1992) from municipalities when they are planning their projects.

This form will be used to evaluate requests to have the Schedule 3 Standards of O. Reg 419/05 - Local Air Quality (the Regulation) made under the EPA apply in advance of the date required by the Regulation

This is for a company to request approval under subsection 35( I) of Local Air Quality Regualtion (O.Reg.419/05) for a Site Specific Standard for a contaminant and a facility.

Organizations obligated to report their compliance under the Accessibility for Ontarians with Disabilities Act (AODA), 2005, including the Ontario Public Service, the Ontario Legislative Assembly, and designated public sector organizations, and businesses and non-profit organizations are required to complete and submit their Accessibility Compliance Report through the Accessibility Compliance Reporting Portal. Under section 2. (1) of O. Reg. 463/24, a person applying for a recovery permit under subsection 152.1 (1) of the Mining Act shall submit an application to the Minister in the approved form.
