Payday Lenders and Loan Brokers Licence Application, and Consent to Disclosure of Personal Information

This checklist is a tool for the Ministry of Economic Development and Growth to make an initial assessment of proposed projects for Youth Skills Connections Industry Partnerships. Consideration of an eligibility checklist does not guarantee acceptance to the next stage of consideration, or funding.

The CSN Referral Screener must be completed by the Service Planning Coordinator prior to referral to the CSN Resource Review process and will assist them in confirming a child’s eligibility. The Civil Remedies Grant Program provides one-time funding of up to $200,000 per project (up to $400,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Seniors; Intimate partner and family violence; Human Trafficking; Gun and gang violence; Hate Crimes; Rural/remote communities.

The purpose of the Private Schools Inspection Report is to provide private schools with a report following a school visit and inspection by the Private School Inspectors. The report identifies issues of non-compliance and provides the school with recommendations on how to improve by their next scheduled inspection.

Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. This licence is required to own and operate a GBHP, including keeping and releasing the authorized species of game birds on the preserve. Applicants must submit a complete licence application with all required supplemental information.

This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Applicants must submit a complete licence application indicating which species of game birds they are seeking authorization to keep before acquiring any birds.

To file an initial return or notice of change by an Extra-Provincial Federal, Domestic or Foreign non-share Corporations under the Corporations Information Act (CIA).

The OAP Workforce Capacity Fund Applicant Checklist is a tool that can be used by applicants prior to submitting an application to the Workforce Capacity Fund to confirm their eligibility to apply for funding. The checklist outlines all eligibility requirements for the Fund, and helps applicants determine if they are eligible to proceed with an application.

The OAP Workforce Capacity Fund Applicant Guidelines provide information on the Workforce Capacity Fund, including the intent of the Fund, eligibility requirements, in-scope and out of scope activities, instructions on how to apply, and ministry contact information for applicant questions.

The use of this aid is voluntary. It is being provided to assist you in maintaining records for medical assistance in dying. Please use this aid if you are a “Medical Practitioner” or “Nurse Practitioner” and a patient is requesting a Waiver of Final Consent. The Waiver of Final Consent is ONLY applicable for individuals whose natural death is reasonably foreseeable (RFND).

The use of this aid is voluntary. It is being provided to assist you in making a written request for medical assistance in dying that complies with the legal requirements. Once you complete this request, you should provide it to your doctor or nurse practitioner. The completed aid may be included in your medical records and may be used by your doctor or nurse practitioner to provide health care to you.

Licensees of Integrated Community Health Services Centres (ICHSCs) must submit an application and obtain written approval from the Director of Integrated Community Health Services Centres prior to a centre’s relocation. For the purposes of this application, a “Fixed Site” centre is a licensed ICHSC where the Limitations and Conditions of the licence specify a single geographic location or address where specified services may be provided.

The Ministry of Health and Long-Term Care's Assistive Devices Program provides customer centered support and funding to Ontario residents who have long-term physical disabilities to provide access to personalized assistive devices appropriate for the individual's basic needs. To accomplish this goal the ADP must establish relationships with health professionals in order to ensure that ADP clients are assessed for cost-effective devices that best suit their needs.

A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.
