A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

This form is used to collect details about the private well, completed by a Qualified Person.
A complete submission includes:

- Ownership documentation
- Four photos of the well from N, S, E, W.
This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions.
The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. Note that a specific form should be used for applications to purchase shoreline reserves or road allowances. The form may be accessed here:

Dairy plant licences must be renewed annually. An application for a licence for the operation of a dairy plant must be submitted at least 60 days prior to the expiry date on an existing licence (s. 95( 4), Regulation 761 - Milk and Milk Products under the Milk Act). If you have recently made any of the following changes to your operation, an application for an Amended licence for the operation of a dairy plant must also be submitted: changes to the classes or subclasses of cow's milk processed at the plant; changes to the species of animal ( cow or goat) that produce the milk processed at the plant; or changes to the type of plant operated.
If the closure plan amendment does not meet the requirements of O. Reg. 35/24, Rehabilitation of Lands, and is being submitted together with a conditional filing order, a certificate in Form 2 to O. Reg. 35/24, Schedule 2, in English or in French, must be signed.
Application may only be completed for recipients of ODSP.
The following approved health care professionals licensed to practice in Ontario can complete the application:
• Physicians, Nurse Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Physician Assistants, and Psychologists*
*Psychologists can only complete the Medical Transportation section of the form.
