A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

Used by beer vendors in Ontario (e.g., licensed establishments) who paid amounts on account of the beer tax on beer made by Ontario beer manufacturers that was sold to a purchaser but for which payment has not been received and the sale has become uncollectible. This does not apply to purchases from or through the Liquor Control Board of Ontario.
Under the Expropriations Act, the expropriating authority must serve this form on registered owners (and may serve the form on owners as defined in s. 1(1) of the Expropriations Act) when a plan of expropriation has been filed in the land registry office but no agreement as to compensation has been made. The form must be served within thirty days after the date of registration of the plan.

Resident trappers are required to obtain the written permission of landowners upon whose land they wish to trap. The Landowner - Trapper Agreement form should be used by trappers to document written permission to trap on private land. Senior Active Living Centre (SALC) programs promote wellness, social interaction, and education to help seniors stay active, independent, and engaged. Ontario will be expanding the SALC Program in 2023-24 and inviting organizations to submit proposals to launch a new SALC program, in a municipality that does not already have an existing SALC program and has a demonstrated need.

The purpose of this Avoidance Alternatives Form (AAF) is to assist the proponent with considering alternatives that would not adversely affect protected species or habitat(s). The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.
