This guide provides information on the EDG for applicants and funding recipients. This Application Guide is valid for the 2021-22 fiscal year. The guide describes the application process as well as project management requirements of the EDG and provides specific information on eligibility criteria and eligible activities. All applications will be assessed, evaluated and approved using requirements outlined in this application guide.

The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. The Program provides one-time funding of up to $100,000 per project (up to $200,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Intimate partner and family violence; Gun and gang violence; and Rural/remote communities.

The form is the Program Guidelines document for the new Improving Connectivity for Ontario (ICON) program, a discretionary, application-based funding program that will support broadband and cellular connectivity in areas of need. The Program Guidelines document provides information about the ICON program rules and conditions and describes the process for applying.

Criminal proceedings in the Superior Court of Justice. Form sets out the conditions upon which an accused is to be released from a detention facility. The form is to be completed by the solicitor of the accused and signed by both the solicitor and the Crown. It accompanies the Release Order of the Superior Court of Justice.

This form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer.

The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.

A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may submit this form with their motion. The form contains a statement by a person who knows the person alleged to be incapable and who has been in personal contact with him or her during the twelve months before the notice of motion was filed.

This form is the prescribed form for a bond of an insurer licensed under the Insurance Act to write surety and fidelity insurance as provided for under clause 14(2)(b) of the Bailiffs Act, R.S.O. 1990, c. B.2 and prescribed as"Form 2" under paragraph 2 of section 2 of R.R.O. 1990, Reg. 53 made under that Act.

A registration is needed if you wish to operate a research, teaching, or testing facility in Ontario. This is required if your premises uses animals in research, teaching or testing and your premises are used for collecting, assembling, or maintaining of animals in connection with a research facility under R.S.O. 1990, c. A.22, s. 4. Please complete this application for registration of a research/teaching/testing facility in full, as per section 4 of the Animals for Research Act. The Director appointed under the Animals for Research Act will approve applications for registrations in accordance with the requirements of the Act. A registration expires on the 31st day of December of the year in which registration is made. Registration must be renewed annually. If an application includes more than one research, teaching or testing facility, details must be provided for each facility in the application, and payment of fees are required.

Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.

Generally, wildlife cannot be kept in captivity in Ontario. There are some exceptions provided through authorization for persons who engage in approved activities. Persons may apply for a Wildlife Scientific Collector’s Authorization to capture, kill, temporarily possess, and/or release wildlife for educational or scientific purposes. This is the application form for a Wildlife Scientific Collector’s Authorization. Applicants must submit this completed application form as well as any required supporting materials. A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.
