Application to Participate in the Lake Simcoe Emerald Shiner Testing Program: Facility Details Form (Part 2)This form (Part 2) is to be used by Commercial Bait Harvesters that have BHAs within the Lake Simcoe watershed and want to have their Emerald Shiners tested for VHS as part of the Lake Simcoe Emerald Shiner Testing Program, which would enable Emerald Shiners that test negative for VHS to be transported out of the Southern Bait Management Zone and into the Central BMZ and the Northeast Testing Program Area. Landowner- Trapper AgreementResident trappers are required to obtain the written permission of landowners upon whose land they wish to trap. The Landowner - Trapper Agreement form should be used by trappers to document written permission to trap on private land. This completed form may be requested by the ministry before the ministry will make any consideration of issuing a resident trapping licence to a person. Information submitted through this form may assist in assignment of harvest quotas for some species.
