This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. The purpose of the Private Schools Inspection Report is to provide private schools with a report following a school visit and inspection by the Private School Inspectors. The report identifies issues of non-compliance and provides the school with recommendations on how to improve by their next scheduled inspection.

This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Applicants must submit a complete licence application indicating which species of game birds they are seeking authorization to keep before acquiring any birds.

This licence allows an individual or corporation to own and operate a Train and Trial Area. Part A of this form contains the application for the licence and Part B contains the required annual report. Those who wish to apply for this licence must have previously been issued a Licence to Own and Operate a Train and Trial Area, applications for new areas will not be accepted. Applicants must submit Part A and Part B of this form, as well as a copy of their site map.

You may use this application form to apply for the Ontario Seniors Dental Care Program if you and your spouse (if applicable) have filed your Personal Tax Return(s) with Canada Revenue Agency (CRA) for the most recent tax year and have a valid Social Insurance Number (SIN). You must keep the confirmation of registration you receive for as long as the animal is in your possession. Submission Requirements: This form is to be used to register: Possession of a carcass or part of a carcass for certain wildlife species regulated under the Fish and Wildlife Conservation Act, 1997. This form is used by mining companies to notify the Ministry of Labour of reportable incidents as identified in section 53 of the Occupational Health and Safety Act and section 21(5) of the Regulation for Mines and Mining Plants. It includes notification of groundfall or rockburst and vehicle incident or fire.
