To collect information regarding work requested to take place on private property without municipal organization and located within a restricted area or for structures proposed on Crown land.

This form is used to collect spatial information to identify the exact location of a private well.

Mechanism to change information (e.g., address) of a designate or remove a designate from a commercial bait harvest licence. Designates are persons 16 years (or older) that are granted the authority to harvest bait on behalf of the licence holder in the absence of the licence holder

Mechanism to apply for a bait harvest area (BHA). BHA's are an allocated portion of the resource and is the scale at which commercial bait harvesters are allowed to harvest baitfish and leeches. A commercial bait harvester can obtain as many BHAs as are available.

This form is to be used by community hatcheries only to apply for an Aquaculture Licence, a Licence to Collect Fish for Aquaculture, and/or a Licence to Stock Fish.

Log book to be kept by the holder of a Licence to Keep Specially Protected and Game Wildlife in a Zoo, as required by Ontario Regulation 668/98: Wildlife in Captivity under the Fish and Wildlife Conservation Act, 1997.

The Fish and Wildlife Conservation Act, 1997, prohibits the sale/purchase of game wildlife or specially protected wildlife, including pelts, except under the authority of a licence and in accordance with the regulations. Section 8 of Ontario Regulation 666/98 (Possession, Buying and Selling of Wildlife) enables a person who holds a Fur Dealer’s Licence to buy or sell pelts. In addition, a Fur Dealer’s Licence is generally required to tan, pluck or treat pelts.

The purpose of this Application Form is to provide MNRF with the necessary information to process and review a request for a new (Type A), revised (Type B) or renewed (Type C) license for Cage Aquaculture Facilities.

This is the Notice of Activity on Public Land and Shorelands form a person must complete and submit to the Minister to satisfy the notice requirements set out in subsection 8 (3) 2 and 5 (2) 3 of Ontario Regulation 239/13 under the Public Lands Act, R.S.O. 1990. for the purpose of maintenance, repair or replacement of erosion control structures on shore lands and construction or placement of buildings within unpatented mining claims.

This licence allows an individual or corporation to own and operate a Train and Trial Area. Part A of this form contains the application for the licence and Part B contains the required annual report. Those who wish to apply for this licence must have previously been issued a Licence to Own and Operate a Train and Trial Area, applications for new areas will not be accepted. In general, you can keep a dead wild animal or bird you have found or been given. In some cases, you need to register that you have a dead wild animal in order to keep it. Registering for a Notice of Possession allows you to keep the dead wild animal for personal use. You must keep the confirmation of registration you receive for as long as the animal is in your possession. Submission Requirements: This form is to be used to register: Possession of a carcass or part of a carcass for certain wildlife species regulated under the Fish and Wildlife Conservation Act, 1997. There is a reporting requirement for activities carried out under a Licence to Collect fish for Scientific Purpose. This form is an electronic version of the mandatory report. The specific regulation requiring this report reads: O. Reg. 664-98 34.1 & O. Reg. 261/05, s. 9.

The form provides a template for Examiner's to provide well information for annual and semi-annual examinations. These reports are legislated through the Provincial Operating Standards. Examinations take place under the Exameriner's Program, which is administered by the Ministry of Natural Resources and Forestry's Petroleum Operations Section. This procedural guidance document is intended for those proponents with renewable energy projects proposed in whole or in part on provincial Crown land who have received a Feed-In Tariff (FIT) 5 energy procurement contract offer by the Independent Electricity System Operator (IESO).

This new, streamlined, AODA compliant form will be available to MNRF staff across the province. This form is used by a variety of MNRF staff to initiate amendments to area-specific land use policy housed within the Crown Land Use Policy Atlas (CLUPA).

This form is required for all escapes or unauthorized releases of cervids (members of the deer family, both native and non-native). The Fish and Wildlife Conservation Act (FWCA) requires cervid owners (person who is keeping the cervids in captivity) to: (1) promptly notify the Ministry of all escapes and unauthorized releases, (2) address the escape/release in coordination with the Ministry, and (3) complete and submit this form (FW4020) If the owner cannot submit the form promptly, they should provide the required details to their Ministry work centre via telephone, or as directed by the work centre.

Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.

A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.
