This form (Part 2) is to be used by Commercial Bait Harvesters that have BHAs within the Lake Simcoe watershed and want to have their Emerald Shiners tested for VHS as part of the Lake Simcoe Emerald Shiner Testing Program, which would enable Emerald Shiners that test negative for VHS to be transported out of the Southern Bait Management Zone and into the Central BMZ and the Northeast Testing Program Area. Applicants need to submit both Part 1 and Part 2 of the application for screening and approval. This form (Part 2) provides details on each of the Holding Facilities identified in Part 1.

Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.

This is the Notice of Activity on Public Land and Shorelands form a person must complete and submit to the Minister to satisfy the notice requirements set out in subsection 8 (3) 2 and 5 (2) 3 of Ontario Regulation 239/13 under the Public Lands Act, R.S.O. 1990. for the purpose of maintenance, repair or replacement of erosion control structures on shore lands and construction or placement of buildings within unpatented mining claims.

Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. This licence is required to own and operate a GBHP, including keeping and releasing the authorized species of game birds on the preserve. Applicants must submit a complete licence application with all required supplemental information.

The Aggregate Resources Act (ARA) requires all licence and permit holders to submit an annual Compliance Assessment Report to the ministry and municipalities. Regular assessment of compliance helps the licensee or permittee to stay familiar with what activities are permitted on their site. Enhanced compliance action will not be taken before the established deadline date for any contraventions disclosed in the report. All information in this report is available for public review. Submit this form and any additional information online using the Natural Resources Information Portal (NRIP). A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. A foster caregiver is a person who acts as an agent under a wildlife custodian authorization by providing supplemental care of wildlife on the foster caregiver’s own premises. A wildlife custodian shall submit to the District Supervisor a completed Foster Care Agreement form for each foster caregiver within 48 hours of the foster caregiver first receiving game wildlife or specially protected wildlife for care and treatment.

This form allows a hunter to apply for a waiver of hunter reporting penalty when serious illness or injury incapacitated the hunter in the 24 hours before the reporting deadline preventing them from completing their report, or when a hunter who was hunting with an outfitter submitted their report but the outfitter didn’t provide the completed report to the Ministry.

Mechanism to change information (e.g., address) of a designate or remove a designate from a commercial bait harvest licence. Designates are persons 16 years (or older) that are granted the authority to harvest bait on behalf of the licence holder in the absence of the licence holder

Mechanism to apply for a bait harvest area (BHA). BHA's are an allocated portion of the resource and is the scale at which commercial bait harvesters are allowed to harvest baitfish and leeches. A commercial bait harvester can obtain as many BHAs as are available.

This form is to be used by community hatcheries only to apply for an Aquaculture Licence, a Licence to Collect Fish for Aquaculture, and/or a Licence to Stock Fish.

The Objection Form is a requirement in section 0.5(9) of O. Reg. 244/97, under the Aggregate Resources Act, to serve as a record of outstanding objections with respect to an application for an Aggregate licence, after the applicant has attempted to address all comments during the consultation period.

Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This application can be used to apply for an Authorization to Conduct Field Trials and Training which may be issued to authorize these activities during certain discrete periods of time outside of the hunting season for the game species being chased.

This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. When notification is required, this form must be circulated with the amendment application package. Detailed submission information can be found on the form, depending on the nature of the amendment.
