The Offset Initiative Registration Application is to be used by sponsors, such as individuals or entities, who wish to register or re-register a single offset initiative pursuant to Ontario Regulation 539/17, Ontario Offset Credits. It is used to provide the Director of the Cap and Trade Branch with information about the offset initiative, including documentation signed and dated by the sponsor.
The Butternut Data Collection Form is the form that must be used by a person who is a Butternut Health Expert (as defined in Ontario Regulation 830/21) when conducting a Butternut health assessment for the purposes of the Endangered Species Act, 2007 (ESA).
Section 7(2) of the Pesticides Act requires a person to hold a permit issued by the Director for a water extermination. This application form is for a permit to use a pesticide to control biting flies or aquatic weeds.
A person who operates an airborne machine in performing a land extermination must hold a permit issued by the Director unless exempt under regulation. This is the application form for this purpose.
This form is part of the Director's Directions for Operational Plans. These directions provide instructions respecting the preparation and content of operational plans prepared by owners of municipal residential drinking water systems pursuant to subsection 16(2) of the Safe Drinking Water Act, 2002, and where otherwise required under the Act. A completed copy of this form is required to be included in every operational plan.
Section 7(1) of the Pesticides Act requires a person to hold a permit issued by the Director for a structural extermination unless exempt under regulation. This application form is for a permit to use a fumigant gas or chloropicrin for any fumigation unless exempt under regulation.

The Drinking Water Systems Profile Information form is used to register and maintain up to date information for Drinking Water Systems subject to Ontario Regulation 170/03.
To ensure the minimum required information identified in the check-list on the reverse of this sheet has been submitted.
This form is used for registering a facility, updating or revoking an existing registration, the identification of a licensed laboratory or to provide notice of reduce lead sampling.
The requirement to submit a Notice of Point of Entry Disclosure for Your Drinking Water Users applies to every Drinking Water System that uses Point of Entry Treatment in accordance with Schedule 3 of O. Reg. 170/03 and belongs to one of the following categories of Drinking Water Systems: • Small Municipal Residential and Non-municipal Year Round Residential systems that serve fewer than 101 private residences.
In accordance with the Drinking Water Systems Regulation (O. Reg 170/03), Schedule 15.1, the following drinking water systems are required to take plumbing and distribution samples and to have the samples tested for Lead by a licensed laboratory: • Large Municipal residential systems • Small Municipal residential systems • Non-municipal year round residential systems.
