The OOC/OOP PA Program eForm is designed to be completed and submitted electronically for application for prior approval for full payment of insured Out-of-Country (OOC) & Out-of-Province (OOP) laboratory and genetics testing services. English and French versions can be completed online or downloaded and saved for future use.

To allow birth parents and adopted persons submit a service request to the Office of the Registrar General to register or withdraw a No Contact Notice under the Adoption Information Disclosure Act, 2005.

To support direct bank deposit enrolment and information management for third party vendor records in Social Assistance Programs

To provide birth parents with options when submitting a service request to the Office of the Registrar General to apply for Post Adoption Birth Information under the Adoption Information Disclosure Act, 2005.

Used by beer vendors in Ontario (e.g., licensed establishments) who paid amounts on account of the beer tax on beer made by Ontario beer manufacturers that was stolen, destroyed (e.g., by fire), or lost (e.g., vehicle accident) prior to the sale of the beer to purchasers. This does not apply to purchases from or through the Liquor Control Board of Ontario.

Prior to beginning alterations to an existing dairy plant, an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant - for Existing Licensed Plants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.

Prior to beginning construction of a new dairy plant an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – for New Applicants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the plant will be built to meet the regulatory requirements under the Milk Act (Ontario). An “Application for a Licence for the Operation of a Plant” must also be submitted with this permit application.
