This form can be used to submit a complaint about a child care program in Ontario that may not be following the rules set out in the Child Care and Early Years Act, 2014 (CCEYA).

The Ministry collects personal information contained in your complaint under the authority to collect personal information, directly or indirectly, in subsection 70 (1) of the CCEYA and subsection 51.1(2) of the Early Childhood Educators Act, 2007 (ECEA). The primary purpose for the Ministry’s collection of this information is to administer and ensure compliance with the CCEYA, the ECEA and their regulations.
Your name and contact information will remain confidential and will not be shared with the child care provider, program, or agency unless required by law.
General details from your complaint (excluding your personal information), such as the nature of the concern, may be shared or discussed with the provider, program, or agency as part of the follow-up process.

A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

Under the Crown Forest Sustainability Act, 1994 and Ontario Regulation 167/95, you need a Forest Resource Processing Facility (mill) Licence if you want to construct and/or operate a mill and your mill consumes more than 1,000 cubic metres of forest resources in one year (regardless of the source). If you have a mill licence, you will need to get a new one where your existing licence is expiring/has expired or your mill: a) changes ownership, and/or b) changes productive capacity, and/or c) is converted to another mill type.
The CSN Referral Screener must be completed by the Service Planning Coordinator prior to referral to the CSN Resource Review process and will assist them in confirming a child’s eligibility. It will also assist in obtaining the appropriate consents from the family and provide them key information about the CSN Resource Review process
Applicants interested in applying for the 2025-26 Community Emergency Preparedness Grant (CEPG) need to review the Application Guidelines. The purpose of the guidelines is to provide applicants with program information including eligible applicants, eligible expenses, report requirements and other useful information.
To allow adopted persons and birth parents to submit a service request to the Office of the Registrar General to register or withdraw a Disclosure Veto under the Vital Statistics Act.

If the rehabilitation measures set out in the closure plan conform to the standards, procedures and requirements of the Part or Parts of the Code to which the certificate relates, the certificate shall be in Form 3 to O. Reg. 35/24, Schedule 2.
The ICHSC Program must be notified of a change in quality assurance advisor through the submission of the Quality Assurance Advisor form which must be signed by both the centre’s quality assurance advisor and the licensee.

If the owner of surface rights considers that a mining claim holder, of the same land, damaged the owners surface rights and is refusing to compensate the surface rights owner, then that owner can send this form to the Mining and Lands Commissioner.
