The application for a meat plant licence includes the completion of a Business Profile. The Business Profile is to be completed by the licensee and reviewed by OMAFRA. It captures necessary information used to assess the types of regulated activities conducted by the operation and the level of inspection services required at the facility. Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.

The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires that a person who keeps specially protected raptors or non-indigenous falconry birds in captivity keep a log book. Log books must be updated within 24 hours of any business transaction or event, or as soon as reasonably possible. Log book records need to be kept for five years and must be accurate.

The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires a person who keeps specially protected raptors in captivity to submit an annual report of all of the specially protected raptors they possessed to the Ministry of Natural Resources and Forestry. The person must also keep a copy of the annual report for five years.

An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario). If these changes require alterations or modifications to a plant you must also submit an “Application for a Permit to Construct or Alter a Building Intended for use as a Dairy Plant - For Existing Applicants”.
