This form is to be used by persons who are eligible to apply and who wish to apply for registration as a voluntary participant in the cap and trade program pursuant to Part IV of O.Reg 144/16 The Cap and Trade Program Regulation made under the Climate Change Mitigation and Low-carbon Economy Act, 2016 (the “Cap and Trade Regulation”).
The purpose of Ontario Community Environment Fund is to channel money collected through environmental penalties into projects located in the tertiary watershed in which the violation(s) occurred. Eligible projects include environmental remediation, research and education relating to spills and restoration of the environment, and projects related to spill preparedness.
This form is part of the Director's Directions for Operational Plans. When a tile drainage machine changes ownership, the Regulations under the Agricultural Tile Drainage Installation Act requires that a notice of transfer of a machine must be completed and submitted within ten days after the transfer occurs. It is illegal to have a loaded firearm in or on, or discharge a firearm from, an aircraft, vehicle (including snowmobile and all-terrain vehicle) or motorboat. This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm. A non-shopkeeper distributor licence is required to sell or distribute fluid milk products if you sell more than 50% by volume of your fluid milk products wholesale and you:
• process fluid milk products
• buy fluid milk products wholesale, or
• operate a shop where you also sell some fluid milk products directly to consumers
Licences are issued for a period of up to three years and must be renewed before the expiry date stated on the licence.

A foster caregiver is a person who acts as an agent under a wildlife custodian authorization by providing supplemental care of wildlife on the foster caregiver’s own premises. A wildlife custodian shall submit to the District Supervisor a completed Foster Care Agreement form for each foster caregiver within 48 hours of the foster caregiver first receiving game wildlife or specially protected wildlife for care and treatment.
This form is an application for a licence to operate a provincial meat plant under the Food Safety and Quality Act, 2001 and O. Reg. 31/05. It is used to apply for a new licence or to revise information on an existing licence.

Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.
A License is needed if you wish to operate a supply facility. Supply facility means premises that are used for breeding and rearing of animals to be used in research, teaching or testing. Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. Licenses must be renewed annually. A separate application and payment of fees is required for each supply.
No person shall make an alteration with respect to a meat plant unless the operator of the plant has given written notice of the proposed alteration to a director at least 30 days before making the alteration.
A licensee of a provincially licensed meat plant may voluntarily surrender their meat plant licence if they no longer conduct regulated activities.
The licensee of a fish processing operation may consent to the revocation of their licence by a director if the operation no longer conducts regulated activities.
