The application for a meat plant licence includes the completion of a Business Profile. The Business Profile is to be completed by the licensee and reviewed by OMAFRA. It captures necessary information used to assess the types of regulated activities conducted by the operation and the level of inspection services required at the facility. It also requires updates and resubmission every three years and when any changes occur to any of the contained information.

A non-shopkeeper distributor licence is required to sell or distribute fluid milk products if you sell more than 50% by volume of your fluid milk products wholesale and you: • process fluid milk products • buy fluid milk products wholesale, or • operate a shop where you also sell some fluid milk products directly to consumers Licences are issued for a period of up to three years and must be renewed before the expiry date stated on the licence.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. It is illegal to have a loaded firearm in or on, or discharge a firearm from, an aircraft, vehicle (including snowmobile and all-terrain vehicle) or motorboat. This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm.
