If the closure plan does not meet the requirements of O. Reg. 35/24, Rehabilitation of Lands and is being submitted together with a conditional filing order, a certificate in Form 2 to O. Reg. 35/24, Schedule 2, in English or in French, must be signed.
Employment Standards Appeal for Licencing
The Early Researcher Award is a discretionary, non-entitlement program that gives funding to new researchers at publicly funded Ontario research institutions to build a research team. Through the completion of this form and supplementary requirements the applicant may be eligible for the review process and award selection. Upon selection, the institution will sign a contract with the Government of Ontario on behalf of the researcher, detailing the terms and conditions for the award.
This form is meant to be submitted by a practicing Ontario physician on behalf of their patients to request consideration of funding for out-of-country health services. Along with the completed application form, submissions must also include relevant medical documentation.
The use of this aid is voluntary. It is being provided to assist you in making a written request for medical assistance in dying that complies with the legal requirements. This is the application for a licence or renewal of a licence to place children for adoption issued by a Director under the Child, Youth and Family Services Act, 2017 (this person is generally the Coordinator of the ministry's Private and International Adoption Unit).
Licensees of Integrated Community Health Services Centres (ICHSCs) must submit an application and obtain written approval from the Director of Integrated Community Health Services Centres prior to a centre’s relocation. For the purposes of this application, a “Fixed Site” centre is a licensed ICHSC where the Limitations and Conditions of the licence specify a single geographic location or address where specified services may be provided.
A licence is required to sell livestock medicines to owners of livestock or to another person acting on the owner's behalf, for the treatment of the owner's livestock under the Animal Health Act, 2009 and the Licences to Sell Livestock Medicines Regulation.
