Form 2 - Applicant Report - Pursuant to the Mandatory Blood Testing Act, 2006 and O. Reg. 449/07 To support implementation of the Mandatory Blood Testing Act, 2006

This form is an application for a licence to operate a provincial meat plant under the Food Safety and Quality Act, 2001 and O. Reg. 31/05. It is used to apply for a new licence or to revise information on an existing licence.

Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.

If the rehabilitation measures set out in the closure plan do not conform to the standards, procedures, and requirements of the Part or Parts of the Code to which the certificate relates, but the non-conformance is only to the extent permitted by a conditional filing order submitted together with the closure plan, the certificate shall be in Form 4 to O. Reg. 35/24, Schedule 2.

An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario). If these changes require alterations or modifications to a plant you must also submit an “Application for a Permit to Construct or Alter a Building Intended for use as a Dairy Plant - For Existing Applicants”.

This tool is to be used by public bodies (identified government agencies) that choose to submit conflict of interest rules or revised rules to the Conflict of Interest Commissioner for approval. These rules should use the language and format of Ontario Regulation 381/07 to the extent possible.

It is illegal to have a loaded firearm in or on, or discharge a firearm from, an aircraft, vehicle (including snowmobile and all-terrain vehicle) or motorboat. This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm. A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.
