This form provides information, direction, and guidance to non-profit organizations interested in applying to the Ministry for Seniors and Accessibility's Seniors Community Grant Program. The Government of Ontario is committed to addressing social isolation and helping older adults stay healthy, safe, and active in their communities through this important grant program.
This form is used by mining companies to notify the Ministry of Labour of reportable incidents as identified in section 53 of the Occupational Health and Safety Act and section 21(5) of the Regulation for Mines and Mining Plants. It includes notification of groundfall or rockburst and vehicle incident or fire.
If the closure plan does not meet the requirements of O. Reg. 35/24, Rehabilitation of Lands and is being submitted together with a conditional filing order, a certificate in Form 2 to O. Reg. 35/24, Schedule 2, in English or in French, must be signed.

Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Service (RPS). To register, you must submit an application form along with the floor plan drawings indicating where the X-ray source will be located. RPS will review the submission and, once you have received confirmation of approval, you can install and use the X-ray source in the workplace.
