If the CVOR client is a Corporation, this application must be signed by one of the business owners/partners/corporate officers. If the CVOR client is a Multi-Party, this application must be signed by both members. If the CVOR client is an Individual, this application must be signed by the individual, a trustee, an executor or administrator of the Estate.

The application for a meat plant licence includes the completion of a Business Profile. The Business Profile is to be completed by the licensee and reviewed by OMAFRA. It captures necessary information used to assess the types of regulated activities conducted by the operation and the level of inspection services required at the facility. It also requires updates and resubmission every three years and when any changes occur to any of the contained information.

This form allows a hunter to apply for a waiver of hunter reporting penalty when serious illness or injury incapacitated the hunter in the 24 hours before the reporting deadline preventing them from completing their report, or when a hunter who was hunting with an outfitter submitted their report but the outfitter didn’t provide the completed report to the Ministry.
If you do not meet a legal requirement at the time of Closure Plan submission, you may request a conditional filing order and submit the request form to the ministry. These orders are granted by the minister.
This form is to be filled out by those applying for funding for the Great Lakes Local Action Fund, a new program that provides funding support for local projects that have environmental benefits for the Great Lakes. The purpose of the form is to collect applicant information including contact and organization information, project details for evaluation purposes, and to obtain a declaration that applicants will comply with applicable laws.
A License is needed if you wish to operate a supply facility. Supply facility means premises that are used for breeding and rearing of animals to be used in research, teaching or testing. Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. Licenses must be renewed annually. A separate application and payment of fees is required for each supply.

To obtain a new business licence, the Regulations under the Agricultural Tile Drainage Installation Act indicate that all businesses must employ a least one Class “A” operator and have access to one licensed machines. To issue a new business licence, an inspection of your operation will be necessary. All business licences are issued for a one-year term expiring March 31; the fee for this licence is $250.00.
The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. The Program provides one-time funding of up to $100,000 per project (up to $200,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Intimate partner and family violence; Gun and gang violence; and Rural/remote communities.

This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm. Under the Crown Forest Sustainability Act, 1994 and Ontario Regulation 167/95, you need a Forest Resource Processing Facility (mill) Licence if you want to construct and/or operate a mill and your mill consumes more than 1,000 cubic metres of forest resources in one year (regardless of the source). If you have a mill licence, you will need to get a new one where your existing licence is expiring/has expired or your mill: a) changes ownership, and/or b) changes productive capacity, and/or c) is converted to another mill type.
Prior to beginning alterations to an existing dairy plant, an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant - for Existing Licensed Plants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.
