This form is used by Municipal Weed Inspectors, Bylaw Officers, etc., to order the destruction of specified noxious weeds or weed seeds under the Weed Control Act. The form sets out the owner of the property, its location, the specific noxious weed species or noxious weed seeds to be destroyed and the timeline in which to do so.
The CSN Referral Screener must be completed by the Service Planning Coordinator prior to referral to the CSN Resource Review process and will assist them in confirming a child’s eligibility. The use of this aid is voluntary. It is being provided to assist you in maintaining records of requests for medical assistance in dying. Please use this aid if you are a “Medical Practitioner” or “Nurse Practitioner” and a patient is requesting medical assistance in dying and it is your intention to provide medical assistance in dying to the patient. You should also include the completed aid in the patient's medical records.
A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.
Used by beer vendors in Ontario (e.g., licensed establishments) who paid amounts on account of the beer tax on beer made by Ontario beer manufacturers that was sold to a purchaser but for which payment has not been received and the sale has become uncollectible. On August 16, 2012, the Government of Ontario launched the Municipal Infrastructure Investment Initiative. Pursuant to subsection 98 (2) paragraph 5 of Ontario Regulation 63/09 under the Pesticides Act, a vendor may sell a pesticide to any person who presents a letter signed by the Director under the Pesticides Act confirming that the person meets the requirements set out in both subsections 83 (2) and 83 (3) of Ontario Regulation 63/09 under the Pesticides Act and is thus exempt from requiring an exterminator licence and a permit in respect to a water extermination.
