This document provides information and instructions to help support anyone who manages Seniors Active Living Centre funding, including the three main administrative components of the program: requesting and managing funding, program delivery, and reporting back to the Ministry for Seniors and Accessibility. The guidelines provide information to support organizations in completing their applications before February 6, 2025, at 5 p.m. ET.

Modular Program – Consent to Disclose Personal Information form ON00518E is to be completed and signed by workers working in modular programs. This form provides the ministry with the consent to release their Modular Program Transcript to employers indicated on the form.
This form is to be filled out by police officers seeking to obtain a warrant to arrest a sex offender for the purpose of having him/her comply with the reporting requirements under Christopher's Law. This form is used if they are seeking a warrant via fax – i.e. a telewarrant.
Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.
