This document provides information and instructions to help support anyone who manages Seniors Active Living Centre funding, including: requesting and managing funding, program delivery, and reporting back to the Ministry for Seniors and Accessibility. The guidelines provide information to support organizations in completing their applications before February 5, 2026, at 5 p.m. ET.
The Fuel, Gas and Tobacco Tax Acts provide that the Minister demand security (usually a letter of credit or surety bond) from designated collectors and most other registrants. A Letter of Credit or Surety Bond must be drawn on an Ontario-based financial institution and contain the terms as presented in the listed forms.
Any person commencing a court proceeding to change the terms of their support order must complete a Confirmation of Assignment form to obtain information about whether their support order is assigned. The ministry will only provide information to the support payor, the support recipient or his/her lawyer. When the form is completed it must be sent to MCCSS, Confirmation of Assignment Unit.
A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.
This Notice only applies to a Non-Municipal Year-Round Residential System that has a raw water supply that is ground water, or that has been deemed, under paragraphs 4 or 5 of Section 2(12), section 2 of O. Reg. 170/03, to obtain water from a raw water supply that is ground water under the direct influence of surface water.
This guide provides information on the RPG for applicants and funding recipients. This Application Guide is valid for the 2022-23 fiscal year. The guide describes the application process as well as project management requirements of the RPG and provides specific information on eligibility criteria and eligible activities. All applications will be assessed, evaluated and approved using requirements outlined in this application guide.
This schedule is to be used by an associated group of corporations who elect to allocate the tax effect from the group's $5,000,000 taxable capital exemption based on the previous calender year's total assets.
This form is the prescribed form for a financial guarantee bond that is a type of security that may be deposited with a court under section 24 of the Repair and Storage Liens Act, and prescribed as"Form 12" under section 12 of O. Reg. 111/18 FORMS made under that Act.
This form is the prescribed form for a waiver of further claim under section 24 of the Repair and Storage Liens Act, and prescribed as"Form 10" under section 10 of O. Reg. 111/18 FORMS made under that Act.
Application may only be completed for recipients of ODSP.
The following approved health care professionals licensed to practice in Ontario can complete the application:
• Physicians, Nurse Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Physician Assistants, and Psychologists*
*Psychologists can only complete the Medical Transportation section of the form.
The application may only be completed for medical supplies, items, and devices listed in the MSN Schedule of Benefits (
https://www.ontario.ca/page/ontario-disability-support-program-health-and-disability-benefits) and items must not be otherwise reimbursed or subject to reimbursement from any other source.
If the holder of a Licence to Own and Operate a Train and Trial Area no longer wishes to operate their train and trial area during the period in which their licence is valid, they may request that the licence be transferred to another individual to own and operate that same train and trial area.
The transferee (proposed new licence holder) and the transferor (current/outgoing licence holder) must complete this Application to Transfer a Licence to Own and Operate a Train and Trial Area form and submit to their local ministry work centre.
This is an interim form and is to be used as a supplement to the 2001 version of the CT23 Short-Form tax return for taxation years ending after September 30, 2001. Once the 2002 version of the Short-Form tax return has been released this Update Form will no longer be required.
This form is meant to be submitted by a practicing Ontario physician on behalf of their patients to request consideration of funding for out-of-country health services. Along with the completed application form, submissions must also include relevant medical documentation.
