PCS102 is to be completed by an Applicant's examining Physician and the information collected is used by the Ontario Provincial Police (OPP) with respect to the Casual/Part-time Policing program. Information collected is for the purpose of initiating an employee file in relation to potential employment with the OPP. The authority for this collection of personal information is the Freedom of Information and Protection of Privacy Act, section 38(2), and the Police Services Act, sections 18 and 43.
Students registered in a training program under the Training and Testing Regulation of the Private Security and Investigative Services Act, 2005 must complete this form. This allows the training entity, the ministry, and the test delivery provider Serco, to share information about a student's eligibility to write the test and apply for a licence. Training entities should distribute this form to their students and provide instructions on how to complete and submit it.
Under subsection 11(3) of Christopher's Law (Sex Offender Registry), 2000, this form is to be filled out by police officers seeking to obtain a warrant to arrest a sex offender for the purpose of having the offender comply with the reporting requirement under Christopher's Law. This form is used if they are appearing in court to seek the warrant.
