A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Subsection 73(1) of the Substitute Decisions Act, 1992 requires two statements. At least one statement must be completed by a designated capacity assessor using Form A. Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Services (RPS).
