The Preauthorized Payment Application for Payors authorizes the Family Responsibility Office to automatically deduct support payments from the support payor's bank account. Critical Technology Initiatives Call for Proposals - Submission Guidelines
The form is used to refer an agency for agency to agency consultation services at CPRI. This service is a one-time consultation specifically designed for clinical discussion only. Agency staff will consult with CPRI’s clinical team for support, feedback, or further recommendations for a child/youth they are currently working with. Clients and/or guardians do not attend. Should further CPRI services be required, regular CPRI intake processes apply and a full intake package must be submitted.
The Inclusive Community Grants (ICG) Program supports local governments, community organizations, and Indigenous groups in considering Ontarians of all ages and abilities during the different stages of community planning and development projects. Through the ICG Program, eligible organizations can receive maximum funding of up to $60,000. This application form is for authorized on-reserve tobacco retailers who pay the Ontario Cigar tax when purchasing cigars from their ministry authorized suppliers. Retailers can apply to the ministry for a refund of the cigar tax paid on cigars sold tax-exempt to qualifying First Nation purchasers.
Used by beer vendors in Ontario (e.g., licensed establishments) who paid amounts on account of the beer tax on beer made by Ontario beer manufacturers that was sold to a purchaser but for which payment has not been received and the sale has become uncollectible.
