This form is used to perform one or more of the following tasks within Employment Ontario Information System - Apprenticeship (EOIS-APPR): • associating existing Employer Subscriber to a newly approved modular program • associating existing Employer Subscriber to existing modular program • activating existing Employer Subscriber with an Inactive modular program status

Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Services (RPS). To register, you must submit an application form along with the floor plan drawings indicating where the X-ray source will be located. RPS will review the submission and, once you have received confirmation of approval, you can install and use the X-ray source in the workplace.

This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Critical Technology Initiatives Call for Proposals - Submission Guidelines

The form is used to refer an agency for agency to agency consultation services at CPRI. This service is a one-time consultation specifically designed for clinical discussion only. Agency staff will consult with CPRI’s clinical team for support, feedback, or further recommendations for a child/youth they are currently working with. Clients and/or guardians do not attend. Should further CPRI services be required, regular CPRI intake processes apply and a full intake package must be submitted.

The Inclusive Community Grants (ICG) Program supports local governments, community organizations, and Indigenous groups in considering Ontarians of all ages and abilities during the different stages of community planning and development projects. Through the ICG Program, eligible organizations can receive maximum funding of up to $60,000. This application form must be used by organizations applying to the 2023-24 Inclusive Community Grants Program.
