This form is for individuals/businesses interested in applying for a new Well Contractor Licence, which is required by anyone engaging in the business of well construction, and expires on March 31 each year following the year of issue. Ontario Regulation 903 under the Ontario Water Resources Act prescribes the licensing requirements and outlines the roles and responsibilities for individuals/businesses and activities associated with wells.
Subsection 50(2) of the Substitute Decisions Act, 1992 lists certain special provisions that may be included in a power of attorney for personal care, but only if the person granting the power of attorney completes this form within 30 days of granting the power of attorney.
This form must be used by a person listed in s. 17(1) of the Substitute Decisions Act, 1992 to apply to replace the Public Guardian and Trustee as an incapable person's statutory guardian of property. The person must also submit a Management Plan (Form 2).
All owners of “lagoons” should make S1 submissions. A “lagoon” maybe subject to a discharge limit or objective that goes beyond the “conventional” parameters listed on the S1 form. In that case, the information would be entered on the S2 form.
