A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

This form must be used by a person listed in s. 17(1) of the Substitute Decisions Act, 1992 to apply to replace the Public Guardian and Trustee as an incapable person's statutory guardian of property. The person must also submit a Management Plan (Form 2).
This form is the prescribed form for a financial guarantee bond under section 44 of the Construction Lien Act.
This form notifies a contractor that the joint owner or owner in common of the premises on which the contractor is contracted to make improvements assumes no responsibility for the improvements.
This form certifies the completion of a subcontract for the supply of services or materials.
This form notifies a landlord that a contractor will make an improvement to the premises of the landlord's tenant.
