The Ontario Research Fund – Research Infrastructure - provides research institutions with funding to help support infrastructure needs such as modern facilities and equipment.

The refund is available to the individual or business that purchased aviation fuel at a tax rate higher than the applicable aviation fuel tax rate for Northern Ontario. The purchase/sale of the aviation fuel was in the boundaries as designated as Northern Ontario. The amount refundable is the difference between the amount of tax paid on the aviation fuel and the tax applicable on aviation fuel in Northern Ontario.

This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. When notification is required, this form must be circulated with the amendment application package. Detailed submission information can be found on the form, depending on the nature of the amendment. A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Services (RPS). To register, you must submit an application form along with the floor plan drawings indicating where the X-ray source will be located. RPS will review the submission and, once you have received confirmation of approval, you can install and use the X-ray source in the workplace.

The use of this aid is voluntary. It is being provided to assist you in maintaining records of requests for medical assistance in dying. Please use this aid if you are a “Medical Practitioner” or “Nurse Practitioner” and a patient is requesting medical assistance in dying and it is your intention to provide medical assistance in dying to the patient. You should also include the completed aid in the patient's medical records.

This form is for licensed well contractors and their assistant technicians (not sooner than after four months of employment with the well contractor) interested in applying for or renewing an Assistant Well Technician Identification Card, which expires 36 months following the date of issue. Ontario Regulation 903 prescribes the licensing requirements and outlines the roles and responsibilities for individuals/businesses and activities associated with wells.

This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. For most residential tenancies, new agreements signed on or after March 1, 2021 must use the updated standard lease, dated December, 2020.

This form is to be used by fee-for-service dental providers to enroll clients into the Emergency and Essential Services Stream of Healthy Smiles Ontario.

To obtain a new business licence, the Regulations under the Agricultural Tile Drainage Installation Act indicate that all businesses must employ a least one Class “A” operator and have access to one licensed machines. To issue a new business licence, an inspection of your operation will be necessary. All business licences are issued for a one-year term expiring March 31; the fee for this licence is $250.00.
