A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. The refund is available to the individual or business that purchased aviation fuel at a tax rate higher than the applicable aviation fuel tax rate for Northern Ontario. The purchase/sale of the aviation fuel was in the boundaries as designated as Northern Ontario. The amount refundable is the difference between the amount of tax paid on the aviation fuel and the tax applicable on aviation fuel in Northern Ontario.
This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. When notification is required, this form must be circulated with the amendment application package.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.
Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Services (RPS). To register, you must submit an application form along with the floor plan drawings indicating where the X-ray source will be located. RPS will review the submission and, once you have received confirmation of approval, you can install and use the X-ray source in the workplace.
