The Public Notice of Application Form in subsection 0.4(1) of O. The Aggregate Resources Act (ARA) provides for the transfer of a licence or permit. A transferred licence or permit will allow the continued operation of the licensed or permitted site under the same terms, conditions and site plan requirements as the original licence or permit. Any changes or amendments to the site plan or licence/permit conditions are a separate process.
Prior to beginning alterations to an existing dairy plant, an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant - for Existing Licensed Plants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.
The CSN Referral Screener must be completed by the Service Planning Coordinator prior to referral to the CSN Resource Review process and will assist them in confirming a child’s eligibility. If the owner of surface rights considers that a mining claim holder, of the same land, damaged the owners surface rights and is refusing to compensate the surface rights owner, then that owner can send this form to the Mining and Lands Commissioner.
This form is to be filled out by judges or justices of the peace if they decided to issue a warrant for the arrest of a sex offender for the purpose of having him/her comply with the reporting requirements under Christopher's Law. This form is a regular warrant.
