A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.
A foster caregiver is a person who acts as an agent under a wildlife custodian authorization by providing supplemental care of wildlife on the foster caregiver’s own premises. A wildlife custodian shall submit to the District Supervisor a completed Foster Care Agreement form for each foster caregiver within 48 hours of the foster caregiver first receiving game wildlife or specially protected wildlife for care and treatment.
Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.
A License is needed if you wish to operate a supply facility. Supply facility means premises that are used for breeding and rearing of animals to be used in research, teaching or testing. Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.
