It captures key information about the child and family, outlines current services and goals, and supports ongoing collaboration among providers. The form is intended to be regularly reviewed and updated by Service Planning Coordinators in partnership with families to reflect changes in needs, monitor progress, and guide service adjustments.
Seniors Active Living Centre (SALC) programs promote wellness, social interaction, and education to help seniors stay active, independent, and engaged.
