Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. Licenses must be renewed annually. A separate application and payment of fees is required for each supply.
For non-emergency, on-farm slaughter, a certificate is required to provide stunning, slaughter and dressing services, perform ante mortem and post mortem examinations and ensure humane animal handling and sanitary dressing.

A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.

The licensee of a fish processing operation may consent to the revocation of their licence by a director if the operation no longer conducts regulated activities.
