Licensees of Integrated Community Health Services Centres (ICHSCs) must submit an application and obtain written approval from the Director of Integrated Community Health Services Centres prior to a centre’s relocation. For the purposes of this application, a “Fixed Site” centre is a licensed ICHSC where the Limitations and Conditions of the licence specify a single geographic location or address where specified services may be provided.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. This form is intended for use by persons required to provide an offence declaration under the Child, Youth and Family Services Act, 2017 or under the Intercountry Adoption Act, 1998.

This form allows a hunter to apply for a waiver of hunter reporting penalty when serious illness or injury incapacitated the hunter in the 24 hours before the reporting deadline preventing them from completing their report, or when a hunter who was hunting with an outfitter submitted their report but the outfitter didn’t provide the completed report to the Ministry.

To obtain a new business licence, the Regulations under the Agricultural Tile Drainage Installation Act indicate that all businesses must employ a least one Class “A” operator and have access to one licensed machines. To issue a new business licence, an inspection of your operation will be necessary. All business licences are issued for a one-year term expiring March 31; the fee for this licence is $250.00.
