This form can be used to submit a complaint about a child care program in Ontario that may not be following the rules set out in the Child Care and Early Years Act, 2014 (CCEYA).

The Ministry collects personal information contained in your complaint under the authority to collect personal information, directly or indirectly, in subsection 70 (1) of the CCEYA and subsection 51.1(2) of the Early Childhood Educators Act, 2007 (ECEA). The primary purpose for the Ministry’s collection of this information is to administer and ensure compliance with the CCEYA, the ECEA and their regulations.
Your name and contact information will remain confidential and will not be shared with the child care provider, program, or agency unless required by law.
General details from your complaint (excluding your personal information), such as the nature of the concern, may be shared or discussed with the provider, program, or agency as part of the follow-up process.

This form is used for eligible participants who wish to participate in Skills Advance Ontario's sector-focused employment and training services. This is a participant application form and attestation. It is a “signature required” form.
The form is used to confirm a physician's qualifications to provide the requested services prior to processing a request to affiliate to a particular ICHSC. The licensee must ensure that the physician has been affiliated to the centre before they begin to provide licensed services.
This SAO Training Placement Agreement must be completed by the individual (the “Participant”) participating in the job placement (the “Training Placement”), the employer providing the training placement (the “Employer”), and the transfer payment recipient overseeing the Training Placement (the “Service Provider”).
The ICHSC Program must be notified of a change in quality assurance advisor through the submission of the Quality Assurance Advisor form which must be signed by both the centre’s quality assurance advisor and the licensee.
This form is required for all escapes or unauthorized releases of cervids (members of the deer family, both native and non-native). The Fish and Wildlife Conservation Act (FWCA) requires cervid owners (person who is keeping the cervids in captivity) to:
(1) promptly notify the Ministry of all escapes and unauthorized releases,
(2) address the escape/release in coordination with the Ministry, and
(3) complete and submit this form (FW4020)
If the owner cannot submit the form promptly, they should provide the required details to their Ministry work centre via telephone, or as directed by the work centre.
