The Annual or Semi-Annual Inspection Declaration is used for:
1) Pick-up trucks and trailers with a registered gross weight over 4,500 kilograms and used for personal use only, or
2) Buses used for personal purposes with a gross vehicle weight rating not over 4,500 kilograms.
This form is for Well Contractors Licence holders who wish to renew their licence; which expires on March 31 following the year of issue; and is required by anyone engaging in the business of well construction. Ontario Regulation 903 under the Ontario Water Resources Act prescribes the licensing requirements and outlines the roles and responsibilities for individuals/businesses and activities associated with wells.

This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program. The Service Description Schedule is part of the Transfer Payment Agreement between His Majesty the King in right of Ontario as represented by the Minister of Health (“the Province”) and the Transfer Payment Recipient. The Transfer Payment Recipient will deliver the programs and services in accordance with the requirements as outlined in this Service Description Schedule document in addition to all conditions and requirements within the Transfer Payment Agreement.
