A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

This form is required to explain to applicants the use of information that they provide to the Fund in order to assess and administer payment out of the Motor Vehicle Accident Claims Fund.
The Annual or Semi-Annual Inspection Declaration is used for:
1) Pick-up trucks and trailers with a registered gross weight over 4,500 kilograms and used for personal use only, or
2) Buses used for personal purposes with a gross vehicle weight rating not over 4,500 kilograms.
This form is the prescribed form for an initial certificate under section 24 of the Repair and Storage Liens Act, and prescribed as"Form 4" under section 4 of O. Reg. 111/18 FORMS made under that Act.
The Aggregate Resources Act (ARA) provides for the transfer of a licence or permit. A transferred licence or permit will allow the continued operation of the licensed or permitted site under the same terms, conditions and site plan requirements as the original licence or permit. Any changes or amendments to the site plan or licence/permit conditions are a separate process.
