Falconry Sponsor Information and CertificationIndividuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.018-fw1021
Application for a Licence to Transport Live Fish Taken from Ontario WatersA licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Form 9 - Statement to Terminate Guardianship of Property by Summary Disposition under Section 73A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Subsection 73(1) of the Substitute Decisions Act, 1992 requires two statements. At least one statement must be completed by a designated capacity assessor using Form A. Crane Operator's Medical CertificateAmendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements.
