This authorization is required for individuals, or corporations, to keep injured, sick or immature game wildlife or specially protected wildlife in captivity for the purpose of rehabilitating or caring for them. Those who are applying for this authorization for the first time should review how to become a wildlife custodian and contact their local ministry work centre before completing and submitting this application form.
Applicants applying for a secondary trapping licence for youth must submit a completed Letter of Consent – Secondary Trapping Licence (Youth) for a new licence or renewal. This form collects signed parental/guardian consent and signed mentor trapper consent and mentor trapper licence number.
This form is restricted to Part-time Per Diem Appointee (PTPDA) to submit their claims for Per Diem Services only. It provides for consistent accounting for PTPDA services at source.
The purpose of the form is for clients/cardholders to authorize the MoF to process a credit card payment or recurring credit card payments for non-tax programs and confirm that any and all credit card charges will not be disputed.
