The purpose of this Avoidance Alternatives Form (AAF) is to assist the proponent with considering alternatives that would not adversely affect protected species or habitat(s). The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.

The refund is available to the individual or business that purchased aviation fuel at a tax rate higher than the applicable aviation fuel tax rate for Northern Ontario. The purchase/sale of the aviation fuel was in the boundaries as designated as Northern Ontario. The amount refundable is the difference between the amount of tax paid on the aviation fuel and the tax applicable on aviation fuel in Northern Ontario.

This form is used by employers of workers in Type 2 or Type 3 asbestos operations. A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may submit this form with their motion. The form contains a statement by a person who knows the person alleged to be incapable and who has been in personal contact with him or her during the twelve months before the notice of motion was filed.

A Management Plan must be submitted when a person applies under s. 17(1) of the Substitute Decisions Act, 1992 to replace the Public Guardian and Trustee as an incapable person's statutory guardian of property (using Form 1). The Management Plan is also required in court applications for the appointment of a guardian of property. The Management Plan sets out the applicant's plan for managing the property of the incapable person.

Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions.
