The Accessibility Standards Advisory Council/Standard Development Committee (ASAC/SDC) was appointed by the Ontario government in 2013 with a mandate to review the Customer Service Standard. The purpose of the form is to assist in the collection of compensation information from designated employers under the Broader Public Sector Executive Compensation Act, 2014 (the Act). Issued in accordance with the Act, the Broader Public Sector Compensation Directive (the Directive) specifies requirements with which designated employers are required to comply to provide compensation information upon written request. This application is to be completed only by carriers who no longer require a CVOR certificate. If the CVOR client is a Corporation, this application must be signed by one of the business owners/partners/corporate officers. If the CVOR client is a Multi-Party, this application must be signed by both members. If the CVOR client is an Individual, this application must be signed by the individual, a trustee, an executor or administrator of the Estate.
