The Vendor Registration Application form is an interactive form that will be completed by an Ontario retailer or supplier of home oxygen therapy who is requesting registration with the Assistive Devices Program.

To file an appointment of an agent for service or revise appointment of agent for service by an Extra-Provincial Foreign corporation with share under the Extra-Provincial Corporations Act (EPCA).
The form is required to initiate a new minor improvement project under the Drainage Act.
Under the Expropriations Act, where land to be abandoned is registered under the Land Titles Act, the expropriating authority registers this form in the proper land registry office. The form is also served on each owner.
