This is the Notice of Activity on Public Land and Shorelands form a person must complete and submit to the Minister to satisfy the notice requirements set out in subsection 8 (3) 2 and 5 (2) 3 of Ontario Regulation 239/13 under the Public Lands Act, R.S.O. 1990. for the purpose of maintenance, repair or replacement of erosion control structures on shore lands and construction or placement of buildings within unpatented mining claims.

Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. This form is used by employers of workers in Type 2 or Type 3 asbestos operations. Under the Farm Implements Act, 1990, registration as a distributor is required for any person, including a manufacturer, who, in the ordinary course of business, sells, consigns or delivers farm implements or parts to a dealer. Any person that meets the definition of both dealer and distributor should register as a distributor/dealer.

The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. The Program provides one-time funding of up to $100,000 per project (up to $200,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Intimate partner and family violence; Gun and gang violence; and Rural/remote communities.

A Management Plan must be submitted when a person applies under s. 17(1) of the Substitute Decisions Act, 1992 to replace the Public Guardian and Trustee as an incapable person's statutory guardian of property (using Form 1). The Management Plan is also required in court applications for the appointment of a guardian of property. The Management Plan sets out the applicant's plan for managing the property of the incapable person.
