The following are the Terms and Conditions for filing with the Ministry of Public and Business Service Delivery (“Ministry”) under the Business Corporations Act, Business Names Act, Corporations Act, Corporations Information Act, Extra-Provincial Corporations Act, Limited Partnerships Act and Not-for-Profit Corporations Act, 2010.
This form is to be used by Public Health Units after dental screening to notify parents/guardians that their child would benefit from preventive dental services. If the parent/guardian can complete the form and return it to the Public Health Unit if they wish to enroll their child into the Preventive Services Only Stream of Healthy Smiles Ontario.
There is a reporting requirement for activities carried out under a Licence to Collect fish for Scientific Purpose. This form is an electronic version of the mandatory report. The specific regulation requiring this report reads: O. Reg. 664-98 34.1 & O. Reg. 261/05, s. 9.

The Preauthorized Payment Application for Payors authorizes the Family Responsibility Office to automatically deduct support payments from the support payor's bank account. This method of payment is only available to payors who do not have a regular source of income that makes support deductions on their behalf (e.g., an employer).
The Ministry of Health and Long-Term Care's Assistive Devices Program provides customer centered support and funding to Ontario residents who have long-term physical disabilities to provide access to personalized assistive devices appropriate for the individual's basic needs. To accomplish this goal the ADP must establish relationships with health professionals in order to ensure that ADP clients are assessed for cost-effective devices that best suit their needs.

Critical Technology Initiatives Call for Proposals - Submission Guidelines
The form is used to refer an agency for agency to agency consultation services at CPRI. This service is a one-time consultation specifically designed for clinical discussion only. Agency staff will consult with CPRI’s clinical team for support, feedback, or further recommendations for a child/youth they are currently working with. Clients and/or guardians do not attend. Should further CPRI services be required, regular CPRI intake processes apply and a full intake package must be submitted.
