Asbestos Work ReportThis form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer.022-12-0091

Well Technician's NoticeThis Notice only applies to a Non-Municipal Year-Round Residential System that has a raw water supply that is ground water, or that has been deemed, under paragraphs 4 or 5 of Section 2(12), section 2 of O. Reg. 170/03, to obtain water from a raw water supply that is ground water under the direct influence of surface water.
