This form acts as either a new application for prospective provincially regulated shortline railway companies and/or as an update form for licensees to notify the Ministry of notable operational changes.

The Annual or Semi-Annual Inspection Declaration is used for: 1) Pick-up trucks and trailers with a registered gross weight over 4,500 kilograms and used for personal use only, or 2) Buses used for personal purposes with a gross vehicle weight rating not over 4,500 kilograms.

This form is used exclusively for the Ministry of Transportation’s Telescopic Lens Program to obtain regular updates to ensure participants continue to meet licensing requirements.

To use a vehicle with a capacity to carry one to nine passengers, this declaration must be completed and submitted at a ServiceOntario centre prior to providing intercommunity passenger transportation for compensation. The driver of the vehicle identified in this declaration must hold at least a G.

Facilitate trailer owners/operators to apply for special permits extending grandfathering provision of non-dump semi-trailers with 4 or more axle or double trailers.
