The Road Safety Community Partnership Program provides funding to a network of provincial and regional non-profit organizations with road safety mandates. The program supports public awareness initiatives that address priority issues such as, aggressive driving, pedestrian safety, distracted driving, impaired driving, and cycling safety. This application form is for those interested in participating in Ontario's Automated Commercial Motor Vehicle (ACMV) Pilot Program.
Form 1 is a prescribed graph grid required by Ontario Regulation 555/06: Hours of Service that a driver uses to indicate which duty status they are in at given times throughout the day.
The form is to be completed by either an optometrist or ophthalmologist specializing in low vision rehabilitation for individuals who wish to drive using a telescopic lens.

The Annual or Semi-Annual Inspection Declaration is used for:
1) Pick-up trucks and trailers with a registered gross weight over 4,500 kilograms and used for personal use only, or
2) Buses used for personal purposes with a gross vehicle weight rating not over 4,500 kilograms.
This form is used exclusively for the Ministry of Transportation’s Telescopic Lens Program to obtain regular updates to ensure participants continue to meet licensing requirements.
Facilitate trailer owners/operators to apply for special permits extending grandfathering provision of non-dump semi-trailers with 4 or more axle or double trailers.
