The Road Safety Community Partnership Program provides funding to a network of provincial and regional non-profit organizations with road safety mandates. The program supports public awareness initiatives that address priority issues such as, aggressive driving, pedestrian safety, distracted driving, impaired driving, and cycling safety. This Guide outlines the details of funding, eligibility criteria and the application process.
Form 1 is a prescribed graph grid required by Ontario Regulation 555/06: Hours of Service that a driver uses to indicate which duty status they are in at given times throughout the day.
Anyone planning to construct on or adjacent to a provincial highway may require a permit from the Ministry of Transportation (MTO). A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.
To use a vehicle with a capacity to carry one to nine passengers, this declaration must be completed and submitted at a ServiceOntario centre prior to providing intercommunity passenger transportation for compensation. The driver of the vehicle identified in this declaration must hold at least a G.
The form is to be completed by either an optometrist or ophthalmologist specializing in low vision rehabilitation for individuals who wish to drive using a telescopic lens.

This form is used exclusively for the Ministry of Transportation’s Telescopic Lens Program to obtain regular updates to ensure participants continue to meet licensing requirements.
