The Death Investigation Oversight Council (DIOC) administers a public complaints process through its Complaints Committee, which reviews complaints regarding death investigations, specifically, complaints against a coroner or a forensic pathologist working in the Province of Ontario. Families may use the DIOC complaint form to submit their complaint to the DIOC Secretariat.
This form is to be used by facilities for the mandatory report to a coroner required when a person dies while resident in a long-term care (LTC) home. (Coroners Act, Section 10(2.1)).

Medical Assistance In Dying (MAiD) Death Report - This form is to be used by Medical and Nurse Practitioners for mandatory reporting to the Office of the Chief Coroner (OCC) of a medically assisted death (MAiD) (Coroners Act, Section 10.1 (1)(2)).
This form is to be filled out by judges or justices of the peace if they decided to issue a warrant for the arrest of a sex offender for the purpose of having him/her comply with the reporting requirements under Christopher's Law. This form is a telewarrant.
Under subsection 11(3) of Christopher's Law (Sex Offender Registry), 2000, this form is to be filled out by police officers seeking to obtain a warrant to arrest a sex offender for the purpose of having him/her comply with the reporting requirement under Christopher's Law. This form is used if they are appearing in court to seek the warrant.
